Carle Health is seeking to add a Wound Care Advanced Practice Provider to join Carle Health in Urbana, IL.

• Clinic hours are Mon – Fri, 8 AM – 5:00 PM along with hospital consults and outreach at our Danville location
• Performs consults, assessments, and follow up for patients in the Wound Healing Center.
• Covering Danville Wound Center and the Hyperbaric Unit is required when needed.
• Work with an experienced physician and dedicated support staff
• Serve as a liaison for the Wound Clinic and other health professionals for services required such as hospitalization and emergency medical services
• The Wound Clinic NP would perform selective and excisional debridement(s)
• Maintain accurate and timely documentation of patient medical records and treatment plans
• Completes consultations and daily rounds on patients admitted to the In-Patient Rehabilitation Unit as needed.
• Serve as an educational resource for nursing and administrative staff

Education Requirements:
Master's degree in Nurse Practitioner or Physician Assistant

Licensure/Certification Requirements:
Current Illinois nurse practitioner licensure or physician assistant licensure
