Swedish Medical Center is looking for a ClinicalSocial Worker \(1\.0, Days\) to work in the Maternal and Fetal Specialty Center at Swedish First Hill in Seattle, WA\.
The Social Worker is responsible for providing medical related social services to patients and families\. These services may include but are not limited to advocacy for patients rights, assessment and care planning related to mental and psychosocial needs\.
In this position you will:
Complete initial assessments on all patients admitted
Develop an individualized treatment plan for the patient's anticipated stay
Arrange post\-discharge planning, linkage with community resources, supportive counseling and crisis intervention with patients and in conjunction with multidisciplinary team members
Coordinate care for patients with the MFM Specialty Care Service whose pregnancies are complicated and where the mother and fetus may be eligible for surgery
Requiredqualifications for this position include:
Master's Degree in Social Work from an accredited school
Licensed Social Work Associate \- Advanced \(LSWAA\) or Licensed Social Work Associate \- Independent Clinical \(LSWAIC\)
2 years experience in a clinical setting
Knowledge of reimbursement regulations for Medicare, Medicaid, and Managed Care
Ability to cover a wide range of knowledge in areas of psychosocial assessment, resource, and referral
Preferredqualifications for this position include:
Licensed Independent Clinical Social Worker \(LICSW\) or Licensed Advanced Social Worker \(LASW\) with the Washington State Department of Health
Social work/discharge planning experience in a rehabilitation unit or acute care hospital
